Photograph Referee

As these cards can be used as proof of age, we have to verify that the photograph you supply is actually of you. If you can provide a passport, photo driving licence or PASS hologrammed card then you do not need to provide a photograph referee.

If you cannot provide one of these documents please ask someone who knows you, who is not a relative or part of your family and is in a full time, professional job ( like a teacher, doctor, nurse, community work, social worker, police officer, or religious leader) to write the following on the back of your photograph:

" I confirm that this is a true likeness of [your full name]"

and get them to sign and date the back of the photograph. They must also complete the photograph referee details at part 5 of the application form (page 3). We will contact all photograph referees personally to confirm authenticity of the details provided.
