
Applying for an Accord Card
Application Forms:
The Accord / National Entitlement Card application form has a unique reference number, and must be made in person at a Customer Access Point or Area Office during normal office hours. For your nearest Customer Access Points or Area Office click on the link Contact Us.
When making your application you will need to bring documentary evidence to prove your address in Aberdeen. You will also require a recent passport sized, head and shoulders camera-facing photograph. Write your date of birth and unique reference number on to the reverse of your photograph. (We regret that scanned photographs are not acceptable).
Please note, if you are aged between 11 and 25 years inclusive you may also need to provide additional photographic evidence. You will need to provide a passport, or photo driving licence, or PASS hologrammed card, or provide details from Photograph Referees.
All new secondary school pupils
If you are a new pupil moving up to a secondary school in Aberdeen City (this includes Primary 6 pupils who visit Torry Academy) and you require an application form, you should ask your Accord Administrator / School Office.
This form should be completed and returned to your Accord Administrator in the school office. If you attend a non local authority secondary school then completed applications can be sent directly to the Accord Office. Contact Us

Replacement cards (lost or damaged).
When an application for an Accord/National Entitlement Card is made, the card is issued to you free of charge. Should you either lose or damage this card and require it to be replaced. If you are a pupil contact your school Accord Administrator. Everyone else should Contact Us.
Replacement Accord / National Entitlement Cards will be provided free of charge. However, we reserve the right to make a charge where frequent requests are made.
